SI Joint Dysfunction is a common but often misdiagnosed cause of low back, buttock and leg pain, seen frequently in office workers, new mothers postpartum, runners, gym-goers and people with prolonged sitting or standing. It occurs when the sacroiliac joint - the joint connecting your spine to your pelvis - becomes stiff, unstable or inflamed, causing altered load transfer from spine to legs. This can cause one-sided pain in the lower back, buttock, hip or groin, sometimes mimicking sciatica.
It develops due to prolonged sitting with crossed legs, leg length difference, poor sitting and standing posture, weak glutes and core, tight hamstrings and piriformis, pregnancy and postpartum hormonal changes, sudden twisting or lifting injury, and lack of pelvic stability. Patients typically report pain on one side of the buttock just above the tailbone, pain while sitting long, standing from sitting, climbing stairs, turning in bed, or walking long, with stiffness in the morning, feeling of leg being heavy, and pain that improves with movement but worsens with staying in one position.
At Physionautics, Vasant Vihar, we have helped 150+ patients with persistent low back and buttock pain become pain-free after being wrongly treated for disc or sciatica. Our assessment includes pain pattern and SI joint provocation tests, pelvic alignment and leg length analysis, gluteal and core stability testing, hip mobility and muscle tightness evaluation, and gait and posture analysis to find whether the joint is stuck or unstable and identify the exact root cause.
Our treatment is personalized and includes pain and inflammation relief with cryotherapy, IFT and TENS, hands-on joint correction with Maitland and Mulligan mobilization, myofascial release and dry needling for piriformis and gluteal tightness, followed by targeted gluteal activation, deep core and pelvic stabilization, SI belt guidance if needed, postural correction and sitting/standing training, and return-to-walk, run and gym program to prevent recurrence.
At Physionautics, we don't just relieve your buttock pain, we correct the underlying pelvic imbalance for long-term, medicine-free and surgery-free relief.
