



De Quervain’s tenosynovitis is a painful condition involving inflammation of the tendons on the thumb side of the wrist. These tendons help in thumb movement and gripping activities. When they become irritated or thickened, pain and difficulty in hand function occur.The most common cause is repetitive overuse of the thumb and wrist. Activities such as frequent mobile phone use, typing, writing, lifting heavy objects, or repeatedly lifting a baby can strain the tendons. Sports that involve gripping and wrist movement, such as golf, badminton, tennis, and pickleball, can also contribute. Poor wrist ergonomics at work, sudden increase in activity, or incorrect hand posture may worsen the condition.Hormonal changes during pregnancy and the postpartum period increase the risk, especially in women. Direct trauma to the wrist, inflammatory conditions, and prolonged holding of the wrist in awkward positions can also lead to De Quervain’s tenosynovitis.Physiotherapy plays a key role in managing De Quervain’s tenosynovitis and preventing recurrence. The first step is pain and inflammation control using rest, activity modification, ice therapy, and modalities such as ultrasound or TENS when required. A thumb spica splint may be advised to reduce strain on the affected tendons.Once pain reduces, gentle stretching exercises for the thumb and wrist are introduced to improve tendon mobility. Tendon gliding exercises help reduce stiffness and improve smooth movement within the tendon sheath. Gradual strengthening exercises for the thumb, wrist, and forearm muscles are added to restore grip strength and functional use of the hand. Ergonomic advice is an essential part of physiotherapy. Patients are educated on proper hand posture, correct lifting techniques, and modification of daily activities to avoid overloading the wrist and thumb. With early physiotherapy intervention, most patients experience significant pain relief, improved function, and a return to normal activities without the need for injections or surgery.
