



Postpartum back pain is one of the most common complaints after childbirth which affects the lower, mid, and even the upper back. This pain can lasts weeks to months. The pain starts from multiple reasons such as pregnancy related hormonal changes, altered posture, weak core muscles, weight gain, and the increased physical demands of feeding, lifting, and carrying the newborn. During pregnancy the hormones relax and loosen ligaments and joints to prepare for birth but this also lowers the stability around the spine and pelvis which makes the back more prone to strain. The growing weight shifts the center of gravity forward which increases lumbar curve and forces the back muscles to work harder also the abdominal wall stretches (diastasis recti), that leaves the spine with less support. After delivery poor ergonomics during breastfeeding, prolonged bedrest or incorrect sleep positions and repeated bending or twisting to care for the newborn further irritate already sensitive tissues and cause pain. Physiotherapy have a role in both treating and preventing persistent postpartum back pain. This is done through restoring stability, mobility, and movement control. Physiotherapy rehab starts with teaching the correct posture and safe body mechanics for activity of daily living like feeding, lifting, and getting in and out of bed because correcting these habits immediately reduces mechanical stress on the spine. Exercises are the next step as in to rebuild deep core, glutes and pelvic floor strength. As the pain diminishes the rehab advances to functional strengthening with hip mobility drills to improve load distribution across the lumber and pelvic region. Manual therapy like soft tissue release, and modalities like heat and TENS/IFT can be used for pain relief but the long term goal is always to help the mother to return to full activity, pain free and prevent chronic back problems through a personalized and progressive rehabilitation program.
